New Patients

Don’t stress about your first visit to one of EJ Therapy’s clinics! The initial examination will take around an hour. It’s important to arrive to the initial exam at your scheduled time as some disciplines will take the full hour. If you have all the paperwork filled out beforehand, our office staff will greet you and make a copy of your insurance card.

Please fill out the online intake forms or print out a copy of the intake forms and bring them to your first appointment. If you forget to fill out the paperwork beforehand, no worries! Arrive around fifteen minutes early and you can fill out the required paperwork at the clinic.
